At Catholic Charities Diocese of Charlotte, our Transition Out of Poverty (TOP) program provides compassionate, customized support to individuals and families striving to overcome poverty.
Through one-on-one case management, participants are empowered to create a sustainable plan that addresses the core challenges often faced on the journey out of poverty, such as:
- Food insecurity
- Employment and income challenges
- Access to affordable healthcare
- Educational opportunities
- Safe and affordable housing
The program’s comprehensive, wrap-around services include:
- Individual Opportunity Plan (IOP): Each participant works closely with a case manager to develop a customized IOP—a road map outlining the steps toward achieving personal and financial stability.
- Scheduled Appointments: Regularly scheduled check-ins with case managers to review progress and adjust the IOP as needed.
- Food Assistance: Immediate access to food resources for those facing food insecurity.
- Job Search Support: Guidance and resources to assist with job searches, resume building, and interview preparation.
- Healthcare Referrals: Assistance with finding and accessing affordable healthcare services and providers.
- Educational Guidance: Help with identifying educational opportunities and resources to improve skills and advance careers.
- Housing Support: Assistance with securing safe, stable, and affordable housing options.
- Application Assistance: Help with completing applications for services and benefits, including public assistance programs.
- Community Resource Connections: Links to local community resources that can provide additional support for specific needs.
- Workshops and Trainings: Opportunities to participate in Catholic Charities-sponsored workshops, trainings, and other personal development programs.
